Data that I collect
If you contact me via my website I will collect your personal identification information, including name, email address and phone number (if we do not end up working together, I will delete this information).
During our sessions together I will take handwritten notes of the information that you share with me. Outside of our sessions together I make digital notes regarding ideas for treatment planning, interventions and further questions I may wish to ask you.
After each session you will be sent an email or text follow up detailing any key points and home practices you have been assigned. There may also be additional email and text communications between us.
How I collect your data
You directly provide me with the data I collect when you:
- Send me an email or text message
- Complete and return your initial consultation form
- Provide me with any personal information during our one-to-one sessions
The data that I hold for you is reasonable and necessary in order for me to provide my services for you.
How I store your data
Your personal details are not shared with anyone else and are stored on a password protected device.
All emails are stored securely on a password protected system. Any emails stored on my phone are protected with face recognition technology.
Handwritten notes taken during our session together do not contain any of your personal identification information, and are kept in a locked filing cabinet. Any digital notes I make and any forms you complete which you send me are stored on a password protected device.
Text messages on my phone are protected with face recognition technology.
How long I retain your data
All information I hold about you has to be kept for 7 years after which time it will be destroyed.
Sharing your data
Some of your personal information may be shared under exceptional circumstances, which include:
- With your GP, or other healthcare professionals
- Requirements of a court of law
- The threat of serious physical harm to either yourself or others
I also undertake group and one-to-one supervision sessions where your case may be discussed; however, you remain anonymous and none of your personal identification details are shared.
Your data protection rights
You have the right to request copies of your personal data.
You have the right to request that I correct any information you believe is inaccurate.
You have the right to request that I erase your personal data, under certain conditions.
You have the right to request that I restrict the processing of your personal data, under certain conditions.
You have the right to object to me processing your personal data, under certain conditions.
You have the right to request that I transfer the data that I have collected to another organisation, or directly to you, under certain conditions.
